Our offices are open from 8:30 am to 5:00 pm, Monday through Friday.
We work collaboratively with your physician to make sure you have a positive experience when you meet with our surgeons. To schedule an appointment, please contact our main office at (510) 886-3138 or (925) 884-2360.
Expect initial appointments to last 30 to 60 minutes.
If you are a new patient, please arrive 15 minutes before your appointment start time so we can complete your registration process. In addition, please bring the following:
- Driver’s license or state ID
- Referral (if required by insurer)
- Insurance information
- Copies of any pertinent medical information and films (X-rays, MRIs, and CT scans from prior doctor visits)
- A list of current medications and any known allergies
Please arrange to bring us actual films, in addition to the written report. The doctor will want to review these films with you. We find that having you bring the films is more reliable than having another medical office or facility mail them.
Follow-up appointments are typically scheduled at the office after your appointment. Otherwise, please call 3-4 weeks in advance to schedule your next appointment.
We make every effort to keep appointments running on schedule, but due to the nature of our specialty emergencies occasionally require us to reschedule. We appreciate your understanding.
There will be a charge for cancellations occurring within 24 hours of your scheduled appointment.
There are over 1,000 insurance companies in the U.S. and over 1,000 different insurance plans. These companies, plans and medical groups are making changes all of the time. There is a good chance that we will not be familiar with covered benefits of each patient’s insurance plan, so please take a moment to become familiar with the specific policies and benefits of your plan.. We believe that health care is a personal matter and appreciate you taking time to know the details of your plan.
Our staff is happy to assist with your insurance billing. Please bring all of your insurance information and your ID card when arrive for your appointment. Also please keep our staff updated with any change of address, phone and/or insurance information.
We will make every effort to determine eligibility, covered benefits and authorization requirements before seeing a patient or ordering a test or procedure, but your insurance may not always pay for all services. Therefore you will be responsible for paying any claims that are not covered. We will ask you to sign an “Informed Consent” notice before you see the doctor or have surgery. This document allows us to bill for any charges that your insurance company does not cover.
We collect co-payments at the time of service. For your convenience we take cash (exact change only), checks and credit/debit cards
If your insurance plan requires a physician referral authorization number please bring that with you or have it called or faxed to us ahead of time. Otherwise we may need to reschedule your appointment.

Medications may be prescribed during the time of surgery.
If pain medication is recommended following surgery, in addition to your usual prescriptions, we will arrange for you to pick up the medication after your surgery date has been confirmed.
Discomfort or pain due to surgery usually resolves within a few weeks. Your surgeon will continue to provide pain medication, if needed, for up to 2 months following surgery. Beyond that, any medication prescriptions should once again be obtained from your primary care or pain management physician.
If you need a refill you can order the medication by logging on to our patient portal at www.pacbrain.com and request your refill by secure email. You can also contact your pharmacy and they can initiate the request electronically. Prescriptions are filled within 3 to 5 days.
Please note that all narcotic prescriptions must be handwritten. Remember to ask for the handwritten script during your post-op visit in order to avoid another trip to our office or additional time for mailing.
If you haven’t been seen by the doctor in 3 months, we will not be able to refill a prescription. Please direct those requests to your primary care physician who should be updated on all medications you are taking.
